If your PS3 never Downgrade any firmware then you can just go to 4.30 v2. If yes then you have to Dehash it
I suggest you install OFW 3.55 first, then go to CFW 3.55kw, Install Newest Multiman 04.13.00 first then CFW 4.30 v2 via Recovery Menu
Be very careful when you install those CFW (especially 4.30 v2), I really suggest to install it via Recovery Menu

Is the game you play have Game Data? If yes then, Delete the Game Data and try to play it again, your PS3 will re-install the Game Data.
Hope this help

Yes dehash first and then recovery update to 3.55OFW.
Then follow this guide from the start, including the dehashing steps.

Make sure you have a game in BD.
You may have to delete game data as Agarest already pointed out.

It is not recommended to go to 3.55kmeaw and then to 4.30, rather one should go to 3.55 rogero v3.7 and then to 4.30cfw rogero v2.
All updates should be through recovery menu.
The tutorial shows this, so the user should dehash and then to 3.55ofw through recovery menu, then they can simply follow this tutorial from the beginning.
EDIT: NOTE: This is an old post from when this tutorial was based on v2.00. Please ignore this post. There are still other posts which are outdated and I'm not able to label them all with such messages, aslong as you follow the relevant tutorial, you'll be fine.

Few things for you to try.
1. Update the game.
You can do this in Multiman by highlighting over the game, then I believe it's triangle and updates.
Multiman will download, after download is finished it will ask whether you want to go back to XMB, go back to XMB and install the update pkgs using 'install package files'.
2. If the first fails, try changing BD Emulator to standard in Multiman.
3. If that fails try going to game settings and selecting BD-mirror.
